If You Have a Physician Referral

If your physician has recommended Speech Therapy, Occupational Therapy, or Lymphedema Therapy, please call our office at 405-564-2701 to schedule your appointment.

To schedule, we will need:

• A physician order (typically faxed directly to us by your provider)

• Your insurance information

You may access our physician order form and downloadable new patient paperwork below to help streamline the process.

Once we receive the referral and required information, we will promptly schedule your comprehensive evaluation.

If You Do Not Have a Referral

Not sure if you need therapy? We’re happy to help.

Call us for a free phone consultation to discuss your concerns. If our therapist determines that an evaluation is appropriate, we will assist you in contacting your physician to obtain a referral if required. Most insurance plans do require a physician order prior to evaluation and treatment.

What Happens Next?

After your referral is received:

1. We verify insurance benefits

2. We schedule your evaluation

3. A personalized treatment plan is developed based on your needs

Insurance & Payment Information

If your insurance plan is not listed above, out-of-network benefits may still apply. Many insurance companies provide partial reimbursement for medically necessary services, although prior authorization may be required.

Our team is happy to assist you in verifying your benefits and determining eligibility. Please contact our office with your insurance information prior to scheduling so we can help estimate coverage and expected out-of-pocket costs.

Payment Policy

 All co-payments, co-insurance, deductibles, and self-pay fees are due at the time of service.

Speech Therapy, Occupational Therapy, and Lymphedema Services are typically eligible expenses under HSA and FSA plans.
